Deborah’s Place

Deborah’s Place breaks the cycle of homelessness for women in Chicago. Through a continuum of housing options and comprehensive support services (health, employment, education, therapeutic and case management services), women achieve their goals of stable housing, sustainable income and greater self-determination. With 187 supportive housing units at three locations within the city, Deborah’s Place is Chicago’s largest provider of supportive housing exclusively for women and is a leader among homeless services agencies locally and nationally.

The award of $5,000 will provide 1000 round-trips on Chicago public transportation which will allow clients to travel to their medical providers to receive coordinated and integrated care as a function of the West Side Collaborative.

Joliet Area Community Hospice Corp.

The mission of Joliet Area Community Hospice is to provide comprehensive, holistic, community based support services and care to terminally ill patients, their caregivers, and their families without regard to economic status.

The award of $5,000 will provide 30 scholarships for their children’s bereavement summer camp.

Lt. Robert E. Hayes and Aaron Hayes Memorial Scholarship and Youth Basketball Foundation

The foundation is a vehicle to perpetuate a memorial for the achievements of Lt. Robert Hayes and Aaron Hayes by serving as an advocate in the community for young men and women with the greatest need. Young men and women will be mentored, guided, and encourage to succeed in their education by graduating from high school, pursuing college and fulfilling the highest potential of their athletic talents.

The award of $5,000 will support the foundation’s endeavor to establish a pre-college program and provide college readiness services to 50 middle and high school students in an effort to create a brighter future for the less fortunate.

Tri City Health Partnership

TCHP provides health care to the uninsured and underinsured. The mission of the clinic includes a dedication to promote better life choices through education and preventative care to foster the growth of the whole individual.

The award of $5,000 will fund the dental care program, which has served 46 patients since its inception in 2009.
